Support threaded output for Discord text channels

Merged Adam "MrBond" Ziegler requested to merge threaded-output into 0.5.x

Adds threaded output to Discord text channel output adapter, if enabled, to these channel types:

  • items - thread per receiving world
  • priority - thread per receiving world
  • hints - thread per sending/containing world

Resolves #86 (closed); also largely resolves #34 (closed).
